We believe it is important to encourage pet owners and guardians to plan for an emergency before one happens. Too often, pet owners and guardians are faced with tough choices about their pets when they are least able to make them. To that end, AMVS provides pre-registration for pet owners so that holidays, vacations, and every-days for your pet can be worry-free!
 
Pre-registration enables you to map out your needs and desires calmly and decisively in advance of any need for a visit to an emergency facility. In addition, the information gathered enables boarding facilities, pet sitters, and anyone else in charge of your pet to act knowingly on your behalf in the event of an emergency. What kind of treatment can the hospital administer? What medications is the pet taking? How much are you willing to spend on treatment or care? What are your wishes if dire circumstances arise? These are all questions to answer before that emergency occurs. Pre-registration with AMVS ensures preparedness for any and every situation with your family member.
 
There is no charge or fee for Pre-Registration. Pre-registration is simply a constructive way to channel your wishes for your pet(s) when you cannot be present. While we sincerely hope that you never require an emergency visit with your pet; as a specialty and emergency facility, we know that visits to us are unforeseen and unplanned. Pre-registration allows both you, the pet owner, and us, the emergency facility treating your pet, to ‘plan’ for the unplanned.


After completing your pet’s pre-registration, we will connect you with a range of materials that can ease your mind if you are traveling with your pet, or vacationing without your four-legged friend! CPR (Cardiopulmonary Resuscitation)/DNR (Do Not Resuscitate)

To allow for optimal treatment of your pet(s), all patients hospitalized at Aspen Meadow Veterinary Specialists/Animal Emergency & Critical Care are assigned a CPR code. This code enables AMVS to carry out your wishes if it becomes necessary during or following an emergency.

If it becomes necessary during your pet's visit/stay, would you like us to administer CPR?

Please note, we will attempt to contact you immediately for further instruction and discussion of your pet's condition if CPR is initiated.

 


Financial Information (optional):

In the event that I am unreachable by phone, I hereby authorize Aspen Meadow Veterinary Specialists, PC (AMVS)/Animal Emergency & Critical Care,LLC (AECC) to do what is necessary to stabilize my pet in an emergency situation.

 

These measures may include (but are not limited to): Radiographs (X-rays), laboratory testing, and injection or pain medication administrations. These initial diagnostic/treatment measures may cost $300-$600. I understand that further testing may be necessary.

 

A thorough exam will be performed by the staff veterinarian. Once you are reached by telephone, you and the veterinarian will create a comprehensive treatment plan for your pet, and an estimate will be prepared. You will be responsible for any charges incurred at the time of service.
