We believe it is important to encourage pet owners and guardians to plan for an emergency before one happens. Too often, pet owners and guardians are faced with tough choices about their pets when they are least able to make them. To that end, AMVS provides pre-registration for pet owners so that holidays, vacations, and every-days for your pet can be worry-free!
Pre-registration enables you to map out your needs and desires calmly and decisively in advance of any need for a visit to an emergency facility. In addition, the information gathered enables boarding facilities, pet sitters, and anyone else in charge of your pet to act knowingly on your behalf in the event of an emergency. What kind of treatment can the hospital administer? What medications is the pet taking? How much are you willing to spend on treatment or care? What are your wishes if dire circumstances arise? These are all questions to answer before that emergency occurs. Pre-registration with AMVS ensures preparedness for any and every situation with your family member.
There is no charge or fee for Pre-Registration. Pre-registration is simply a constructive way to channel your wishes for your pet(s) when you cannot be present. While we sincerely hope that you never require an emergency visit with your pet; as a specialty and emergency facility, we know that visits to us are unforeseen and unplanned. Pre-registration allows both you, the pet owner, and us, the emergency facility treating your pet, to ‘plan’ for the unplanned.

After completing your pet’s pre-registration, we will connect you with a range of materials that can ease your mind if you are traveling with your pet, or vacationing without your four-legged friend! CPR (Cardiopulmonary Resuscitation)/DNR (Do Not Resuscitate)

To allow for optimal treatment of your pet(s), all patients hospitalized at Aspen Meadow Veterinary Specialists/Animal Emergency & Critical Care are assigned a CPR code. This code enables AMVS to carry out your wishes if it becomes necessary during or following an emergency.

If it becomes necessary during your pet's visit/stay, would you like us to administer CPR?

Please note, we will attempt to contact you immediately for further instruction and discussion of your pet's condition if CPR is initiated.


Treatment Authorization and Information/Photo Release

I am the owner, or the agent of the owner, of the above-described pet and have the authority to execute this agreement. I authorize Aspen Meadow Veterinary Specialists, PC (AMVS) and/or Animal Emergency & Critical Care, LLC (AECC) to examine and treat the above pet. I have read and agreed to the financial policy of AMVS and AECC. I accept full financial responsibility for the pet. I understand that payment for diagnostic tests and treatment that I authorize in writing or verbally will be due at the time my pet is dismissed from the hospital.


If another veterinarian has referred me to AMVS, I understand that they will receive a summary of the care and treatment provided by AMVS and/or AECC in order to ensure that my pet’s care can be continued without interruption. I also understand that AMVS and AECC consider the identification of a referring veterinarian by me to be my authorization to release records and information to that veterinarian.


Case information and/or photos may be used in teaching, continuing education, and veterinary literature. I authorize release of case/patient information for such purposes. Patient confidentiality will be maintained. In the event of ownership transfer, I authorize the release of medical information to the new owner of this animal.

AMVS/AECC Financial Policy


We at AMVS/AECC understand that this may be a difficult time for you and your pet. We realize a visit to our clinic is unplanned and the associated fees are unexpected. We would like to offer the following options to assist you:



Cash payments are gladly accepted.


Personal Checks (NO BUSINESS CHECKS)

We accept personal checks only with a current driver’s license. A fee of $25.00 will be assessed on all returned checks. When you provide us with a check as payment you authorize us to either use information from your check to make a one-time electronic fund transfer from your account or to process the payment as a check transaction.


Credit Cards

We accept: VISA, MasterCard, American Express, and Discover* (* denotes AECC only)


Care Credit

CareCredit is a credit card dedicated to health care. It involves a no cost application, which can be processed immediately. We offer a 6 month no interest payment plan. Please ask one of our staff for additional information or an application, if interested.



CitiHealth Card, much like CareCredit, is a line of credit that can be used for various health care visits, human and animal alike. The application can be processed in-house at no cost to you. There are several payment option plans available. Please ask one of our staff for additional information or for an application, if interested.


Veterinary Pet Insurance

We work with all Veterinary Insurance Providers, and can supply you with any information or records you require in order to obtain reimbursement of funds spent on your pet during his/her AECC visit.


Payment in full is due at the time of services. For hospitalized cases, a deposit is required in advance. The balance is due upon discharge from the hospital.
